I understand that photos may be taken during camp and I give permission for photos taken of my child to be used by the South Academy of International Languages, CMS, or the SAIL PTSO in any marketing materials, including but not limited to their websites, social media, and printed materials.


I understand I will need to provide proof that my student attending this camp has seen a physician some time in the past year. Rising 5th and Rising 6th graders only need to email Coach Jones a screenshot of evidence they have had a doctor visit. Rising 7th - Rising 9th graders' caregivers need to provide a completed physical from some time this past year. Dragonfly is not required for this camp. Email Coach Jones these materials by July 7th.
